Root Cause Failure Analysis (RCFA)

Root cause analysis (RCFA) is in the class of problem solving methods aimed at identifying the root causes of problems from an event. The practice of RCFA is to analyze failed components to identify the root cause and therefore to correct or eliminate root causes, as opposed to merely addressing the immediately obvious symptoms. By directing corrective measures at root causes, it is hoped that the likelihood of problem recurrence will be minimized.

Monition Ltd has its own lab facilities and working with key partners access to a range of metal and chemical testing suites for non destructive testing. This enables detailed analysis of parts and materials to take place in order to determine the root causes.

The general process for performing and documenting an RCFA-based Corrective Action is shown below.

1. Secure Evidence from Component failure (Monition to advise)
2. Gather data/evidence
3. Identify issues that contributed to the problem
4. Carry out relevant visual or physical tests
5. Find root causes
6. Develop solution recommendations
7. Implement the recommendations
8. Observe the recommended solutions to ensure effectiveness
